Asmanex Twisthaler is a medicine that is prescribed as a maintenance treatment of asthma in patients ages 4 and older.[1] Asmanex Twisthaler is not a bronchodilator and is not indicated for use in treating sudden asthma attacks.[1] The main ingredient in Asmanex Twisthaler is mometasone furoate inhalation powder, a man-made corticosteroid. Manufactured by Merck, Asmanex Twisthaler is available in two dosing strengths: 110 mcg for patients aged 4-11 and 220 mcg for patients 12 years and older.[1]
Potential Asmanex Twisthaler side effects include nasal allergy symptoms, sore throat, and headache. There are trace amounts of lactose, which contains milk proteins, in Asmanex Twisthaler. Incidences of anaphylactic reactions in patients with milk protein allergies have been reported in post-market research.[2]

Common Asmanex Twisthaler Side Effects
The following side effects occurred in clinical trials with an incidence of at least 3 percent greater than the placebo group.[2]
- Abdominal pain
- Back pain
- Fever
- Muscle pain
- Nasal allergy symptoms
- Oral fungal infection (Candidiasis)
- Painful menstruation
- Sinus infection
- Sore throat
- Upper respiratory infection
- Upset stomach
Other symptoms that were reported at a rate of less than 3 percent but were more common than among the placebo group include:[2]
- Anorexia
- Bleeding of the nose
- Body pain
- Dry throat
- Earache
- Fatigue
- Flu-like symptoms
- Gastroenteritis
- Respiratory disorder
- Vomiting
About Drug Side Effects
Asmanex Allergic Reactions
The use of Asmanex Twisthaler is contraindicated for patients with a known hypersensitivity or allergy to mometasone furoate or to anhydrous lactose.[4] Patients with milk protein allergies may experience anaphylactic reactions due to trace amounts of lactose in Asmanex Twisthaler. Patients who have trouble breathing, develop a rash or itchy skin, or have swelling of the face, mouth, or tongue may be experiencing an allergic reaction. Such reactions could be serious and may require medical attention.[2]
Asmanex Precautions and Warnings
- Asmanex Twisthaler is known to cause cataracts, glaucoma, and increased intraocular pressure in patients with a history of eye disease.[2]
- Asmanex Twisthaler can cause bronchospasms and wheezing upon inhalation. If this occurs, patients may require immediate treatment with a fast-acting bronchodilator, and Asmanex Tqisthaler may need to be discontinued.[2]
- Asmanex Twisthaler can cause a reduced growth rate in pediatric patients. The manufacturer suggests administering the lowest dose possible to effectively control symptoms while minimizing potential growth deficiencies.[2]
- Asmanex Twisthaler can cause a fungal infection of the mouth and throat. Patients are advised to rinse their mouths after using their Twisthaler.[2]
- Corticosteroids, such as Asmanex Twisthaler, are known to cause a reduction of bone mineral density (BMD), especially in patients who are already at risk for decreased BMD.[2]
- Patients on corticosteroids, such as Asmanex Twisthaler, have an increased risk of infection because their immune systems are weakened. Infections such as chicken pox, measles, TB can become serious, even fatal. Treatment with prophylaxis may be suggested if the patient is exposed to one of these infections.[2]
- Asmanex Twisthaler is classified as FDA pregnancy category C, which means that animal reproduction studies have shown an adverse effect on the fetus, yet there are no adequate and well-controlled studies in humans. Reproductive studies using rats, rabbits, and mice showed an increase of fetal malformations. Conditions such as cleft palate, delays in ossification, low fetal birth rate, and dystocia (difficult or complicated birth) demonstrate a potential danger if used during pregnancy. However, asthma is a serious condition which, when poorly controlled, carries its own potential harm for both mother and fetus. Therefore, the decision to use Asmanex Twisthaler while pregnant should be made on an individual basis.[2]
- It is not known if Asmanex Twisthaler can pass through breast milk in nursing mothers. Other corticosteroids have been shown to pass through breast milk, therefore caution should be used when administering Asmanex Twisthaler to women who are nursing.[2]
