Bristol-Myers Squibb offers free prescription drugs to qualified patients through the Bristol-Myers Squibb Patient Assistance Foundation, Inc., a non-profit organization. To apply for the program, patients can download the application form from the official Bristol-Myers Squibb Patient Assistance Foundation website. If patients have additional questions, or want to receive a faxed copy of the application, they can call 800-0736-0003.[8]

In order to be eligible for the Bristol-Myers Squibb Patient Assistance Foundation program, patients must not have either public or private prescription drug insurance coverage, and must meet the following income requirements:[9]

  • Families of 1: Must earn less than $27,225 in the Continental U.S., less than $34,000 in Alaska and less than $31,350 in Hawaii
  • Families of 2: Must earn less than $36,775 in the Continental U.S., less than $45,950 in Alaska and less than $42,325 in Hawaii
  • Families of 3: Must earn less than $46,325 in the Continental U.S., less than $57,900 in Alaska and less than $53,300 in Hawaii
  • Families of 4: Must earn less than $55,875 in the Continental U.S., less than $69,850 in Alaska and less than $64,275 in Hawaii
