Fosamax is a prescription medication used to treat bone loss from osteoporosis and Paget's disease. Fosamax can slow the progression of osteoporosis, but also poses health risks. The most common side effects are benign, and include upset stomach, bowel issues, bloating, changes in (or loss of) taste, headache, and swelling.[1]
Fosamax side effects can cause significant medical problems, especially if the medicine is not taken exactly as directed. Occasionally, osteonecrosis, or "jawbone death" can occur from Fosamax use. Some experts have also found a possible link to esophageal cancer. There is more than one lawsuit against Fosamax pending for severe adverse reactions.[2]

Common Fosamax Side Effects
While side effects of Fosamax are generally mild, they can include abdominal pain, diarrhea, bone pain, muscle cramps, ulcers, headaches, dizziness and altered taste.
A rare but significant side effect of Fosamax can be ONJ, or osteonecrosis of the jaw. Patients needing chemotherapy or significant dental work (including tooth extraction) are advised to discuss the procedure with their doctor prior to any treatment. Some patients may need to suspend Fosamax treatment to prevent ONJ.[2]
Another potential side effect is cancer of the esophagus. The correlation has not been fully established, but the FDA warns that there may be a link. Most esophagus issues occur when Fosamax is not taken correctly.[1]

Fosamax Allergic Reactions
The incidence of allergic reaction to Fosamax is low, but patients who experience any allergies while taking the medication are advised to seek immediate medical attention. Potential allergic reaction symptoms include difficulty breathing, rash, dizziness, and itching or swelling. Patients are advised to discuss any known drug allergies before taking Fosamax.[3]
Fosamax Precautions and Warnings
- There are precise instructions for taking Fosamax, and failing to adhere to them can result in significant adverse reactions. Patients are instructed to take Fosamax with water only, and not consume any food, other pills, or supplements until at least 30 minutes afterwards. in addition, it is important to remain in an upright position (not laying down) for at least 30 minutes. Patients who are unable to sit or stand in an upright position for that length of time are not typically prescribed Fosamax.[2]
- Patients with poor oral health may be at a greater risk for developing osteonecrosis on Fosamax.[3]
- Fosamax is not indicated for use in children. Children who were given Fosamax exhibited flu-like symptoms such as high temperature and upset stomach.[3]
- Fosamax is not recommended for patients with severe renal failure.[3]
- Fosamax is not for patients with abnormalities of the esophagus.[3]
- The effects of Fosamax on pregnancy are not known. It remains in the body for years, and women planning on future children are advised to discuss risks with their physicians.[3]
