Topamax Patient Assistance Programs

The primary patient assistance program available for Topamax offers free Topamax to qualified individuals for up to a year, and is only provided to patients who have no other source of prescription drug coverage available to them.[1]

Eligibility for the Topamax patient assistant program is based on the following:[3]

  • Patients must reside in the United States, Puerto Rico, or the U.S. Virgin Islands
  • Patients must be treated by a licensed U.S. healthcare provider
  • Patient must not have prescription insurance coverage
  • Must have an annual income of $21,660 or less for individuals and $29,140 or less for a family of two.

In order to enroll in the program, patients can either download the application online through the foundation's official website, http://www.jjpaf.org/eligibility/index.html, or contact a foundation representative by calling 800-652-6227. Patients contacting the foundation by phone will receive the application by fax or by mail.[4]

Once a patient has downloaded or received the application, they must fill it out and gather all the required documents, including a copy of the patient's most recent federal tax return. The patient must then take the application and all documents to their healthcare provider to complete. The provider will then need to fax the patient's completed application to 888-526-5168, or mail the application to:[4]

In addition to patient assistance programs, patients may also be able to obtain free samples of Topamax from their doctor. Johnson & Johnson and other pharmaceutical companies provide doctors with samples of the company's new and existing medications.

Topamax Information

Topamax is a prescription medication used in the treatment and prevention of migraine headaches and seizures. The anticonvulsant medication was first approved by the FDA in 1996, and is manufactured by Ortho-McNeil-Janssen Pharmaceuticals, Inc. Using Topamax, the brand name for topiramate, may result in such common side effects as headache, dizziness, general feeling of tiredness, loss of appetite, difficulty sleeping, or problems with speech or balance.[2]
